Notes - Here are the aims of the CIC in full:
“We want improved health and social outcomes for people in the area.
We want all the people in the areato have access to local integrated health and social care services.
We want people to be able to access beds close to home for rehabilitation and step up step down care.
We want to improve community involvement in local health and social care.
We want people to be able to remain in their own homes for as long as possible rather than being admitted to care homes or bed blocking in hospital.
We will seek a plan which will be sustainable for the foreseeable future.”
